This image presents the event more as a vision than as a miracle. The host in Gregory's hands continues to be a host while he casts his gaze up to a cloud-framed vision of the Man of Sorrows surrounded by various details from the Passion narratives. The otherness of the vision is heightened by the contrast in color and detail with the framing narrative of Gregory and his deacons in contemporary dress at a contemporary altar.

Like other Mass of St. Gregory images, this one includes two deacons attending the pope and a number of bystanders, one of whom holds Gregory's tiara.
